Arthur killed his mother as he realized (after reading her report from Arkham State Hospotal) she was the reason for his suffering as a child, she was a delusional woman and was not his biological mother.
Much like her son, Penny suffered from an unspecified mental illness. She was responsible for letting her former boyfriend abuse Arthur in his childhood and was temporarily sent to the Arkham State Hospital for unsolicited claims.

Was Arthur hallucinating Sophie?

Missing Eye: Joker had his right eye shot out by Harley Quinn.
According to the 2017 documentary I Am Heath Ledger, the actor's prosthetic scars, which were caked in red paint, kept coming loose on set. Ledger's habitual licking began as a reflex to check the scars were still in place and to ensure they stayed in position.

It was never fully revealed what the exact cause for the creation of the infection was other than it originating from Joker's mutated blood. However, it was probable that the chemicals that caused the Titan Disease (and maybe chemicals already in the clown's blood) caused Joker's blood to mutate creating the infection.
